Our Targeted Case Management services are designed to assist individuals facing behavioral health challenges by providing personalized support and guidance. We are dedicated to empowering our clients by facilitating access to the necessary medical, social, and educational resources to improve their quality of life.

Program Description

The Targeted Case Management program offers comprehensive support to individuals with behavioral health challenges, focusing on enhancing their ability to navigate various systems of care. Key offerings include:

  • Personalized Planning: Development of individualized service plans tailored to meet the specific needs and goals of each client. This is accomplished by performing a comprehensive needs assessment upon intake, updated every ninety (90) days.
  • Resource Linkage: Assistance in connecting clients with essential medical, social, educational, and other resources including housing, transportation, and community supports.
  • Continuous Support: Ongoing monitoring and adjustment of service plans to ensure they remain effective and responsive to the client’s evolving needs.
