Getting Started

Starting treatment with CommonHealth Recovery is as easy as 1, 2, 3. 

All of our clients begin the CommonHealth Recovery Medication-Assisted Treatment program with three (3) assessments to determine all of your behavioral health needs.

Assessment #1: Care Navigation Assessment (Case Management) (30 Minutes)

We will begin by introducing you to a trained and qualified CommonHealth Recovery Care Navigator to gain access to the necessary medical, social, educational, employment, and recovery support services needed to sustain meaningful recovery. 

Assessment #2: Biopsychosocial Mental Health & Substance Use Assessment (30-60 Minutes)

The biopsychosocial assessment performed by a certified or Licensed CommonHealth Recovery Counselor is a comprehensive evaluation process that considers biological, psychological, and social factors to understand an individual’s physical and mental health, as well as their social environment, in order to guide treatment planning and intervention throughout treatment.

Assessment #3: Medication-Assisted Treatment (MAT) (30 Minutes)

A qualified and licensed CommonHealth Recovery Prescriber will assess your needs for medications that reduce the urge for cravings (Suboxone, Sublocade, Subutex, Vivitrol) and keep you in sustained recovery through a timed, level-up phased approach. Clients who participate in the CommonHealth Recovery MAT program obtain the skills necessary to take medications as prescribed, within ongoing maintenance and monitoring to include drug testing. A prescription will be written on the first appointment, with follow up in 7-days. 

    Clients entering the CommonHealth Recovery Medication-Assisted Treatment Program in Frankfort, KY will meet with their assigned Counselor, Care Navigator, and Prescriber every follow-up visit. Each visit is designed to last one (1) hour.  

    Phase One (Weekly Visits for 8-Weeks):

    • One (1) Counseling Session (30 minutes) Per Week
    • One (1) Care Navigator Visit (15 Minutes) Per Week
    • One (1) Prescriber Visit (15 Minutes) Per Week

    Phase Two (Bi-Weekly Visits for 16-Weeks):

    • One (1) Counseling Session (30 minutes) Every Two Weeks
    • One (1) Care Navigator Visit (15 Minutes) Every Two Weeks
    • One (1) Prescriber Visit (15 Minutes) Every Two Weeks

    Phase Three (Monthly Visits) (Ongoing Maintenance):

    • One (1) Counseling Session (30 minutes) Per Month
    • One (1) Care Navigator Visit (15 Minutes) Per Month
    • One (1) Prescriber Visit (15 Minutes) Per Month
