Step 1: Schedule Your Assessments
The first step in joining CommonHealth Recovery is to call us to schedule your initial assessments. Every new client undergoes two thorough assessments to ensure that we understand all aspects of their needs:
Biopsychosocial Assessment
This assessment covers a wide range of factors, including your mental health, medical health, physical health, substance use history, and family history. Our goal is to gain a comprehensive understanding of your situation so that we can tailor your treatment plan to best support your needs.
Case Management Resource Assessment
After the biopsychosocial assessment, you’ll meet with a case manager to evaluate your needs in areas like medical care, transportation, employment, and housing. This ensures that you have access to the resources you need to support your recovery.
Step 2: Medical Assessment (If Needed)
If your assessments indicate a need for medication, we’ll schedule a third assessment with one of our medical providers. This session is focused on prescribing medications for opioid use disorder, alcohol use disorder, other substance use disorders, and mental health conditions. Our providers work closely with you to determine the most effective medication plan, ensuring a balanced and safe approach to your recovery.
Step 3: Personalized Treatment Planning
Once the assessments are completed, your counselor will collaborate with you to create a personalized treatment plan that aligns with your needs and goals. As an outpatient-only facility, we offer the flexibility for clients to continue with their daily lives while receiving the support they need.
